- Transcript of Records (TOR)
- School's Admission Test Result
- Certificate of Good Moral Character (from 3 officials)
- Biodata with 2 pcs latest 1x1 picture
- National Medical Admission Test (NMAT) Result
- Physical Examination Report & Medical Certificate (CBC, Chest PA, Urinalysis, Stool)
- Latest Income Tax Return of the family
- Barangay Clearance or Certificate of Residency
- Drug Test Result
- NBI Clearance
POST GRADUATE INTERNSHIP
- Certified true copy of Medical School Diploma
- Transcript of Records
- Certificate of Internship Matching (APMC)
BOARD EXAMINATION
- Certificate of Completion (APMC affiliate hospital)
- Certified true copy of PRC Application for Medical Exam
